HCAHPS Expanded Survey "About You" Questions (A resource exclusively for Studer Group Partners)
CMS has recently extended an offer to organizations on a volunteer basis to use an expanded survey beginning with July 1, 2012 discharges. Three of these questions pertain to the care transition and two around patient information, specifically emergency room admission and mental or emotional health.
